REGISTRATION REQUIREMENTS:

1. Completed Registration Form (Online)

2. Child’s birth certificate (original or a “certified to be true” copy with a raised seal) 

3. Proof of Residency: 

As per the New Jersey Department of Education, The New Milford Public School District shall accept a variety and combination of documents in order to show where a student lives and is entitled to attend school.  Please provide the following to register your child in the District:

Supply One:   

 -Original deed/lease      OR 

 -Mortgage statement or tax bill (if you do not have a deed or a lease, affidavits are available at the school's main offices) 

Supply Two (Current):   

-Different Utility bills, (i.e., PSE&G, United Water, Cablevision)       AND/OR  

-Bank Statement and Canceled Check (please cross out account info)

Supply One:

-New Jersey Photo Driver License, New Jersey non-Driver License Card, Passport or State ID with photo

4. Medical Information:
